Here at Andes Central School, students participation in interscholastic athletic competition is encouraged. While we do not offer all sports here, many are available to students through other schools with which we merge interscholastic athletic teams.

On this page you will find the forms necessary should your child wish to participate in interscholastic athletics.

Concussion Information for Students & Parents
This informational sheet is a valuable resource for students & parents about concussions.
Physical Exam Form
Required annually for interscholastic athletic competition.
Initial Athletic Health History
This form is required and must be completed and returned prior to an athlete's annual interscholastic sports physical being given.
Interval Athletic Health History
This forms must be completed prior to the beginning of each interscholastic sports season unless a complete physical was completed within 30 days prior to the start of the season.
